Understanding Medicare IRMAA
What is IRMAA?
IRMAA (Income-Related Monthly Adjustment Amount) is an extra charge added to your Medicare Part B and Part D premiums if your income exceeds certain thresholds.
How is IRMAA Calculated?
- Based on your Modified Adjusted Gross Income (MAGI)
- Uses tax return from 2 years prior (2023 for 2025 premiums)
- MAGI = AGI + tax-exempt interest + certain foreign income
Life-Changing Events
You can appeal IRMAA if you experienced a qualifying life event that reduced your income:
- Marriage, divorce, or death of spouse
- Work stoppage or reduction
- Loss of income-producing property
- Loss of pension income
Strategies to Reduce IRMAA
- Roth conversions before Medicare enrollment
- Timing of capital gains realizations
- Qualified Charitable Distributions (QCDs)
- Tax-loss harvesting
- Managing Required Minimum Distributions (RMDs)
Payment
IRMAA is automatically deducted from your Social Security benefits. If you dont receive Social Security, youll be billed directly by Medicare.
